Do I need a Doctor’s referral?
No, you are able to see your doctor of physical therapy directly without a referral. If you have a provider that would like to send a referral, we are happy to accept referrals and attach them to your medical record. Referrals can be faxed to us at 612-808-5192.
What conditions do you treat?
We specialize in treating complex neurologic and vestibular conditions. Some diagnoses we commonly see are dysautonomia, postural orthostatic tachycardia syndrome (POTS), dizziness/vertigo, concussion, brain injury, long Covid, stroke, multiple sclerosis, Parkinson's disease, and Guillain-Barre Syndrome. What types of health insurance do you accept?
We are able to accept patients from most insurance plans, and are considered an out of network provider. Please schedule a complimentary discovery call with us to discuss the details.
What should I bring to my appointment?
Please wear clothing that you feel comfortable exercising in. You are welcome to bring a water bottle with, but water bottles are not required.
Do you offer virtual appointments?
Yes, virtual appointments are available and can be scheduled by calling 612-284-1970.
What are your COVID policies?
We are following all state and federal regulations, and are frequently updating our practices to reflect the most up to date information available from the CDC and other organizations. Please contact us with questions concerning our current policies as they are frequently changing to keep patients and providers as safe and healthy as possible.
Where are you located?
All in-person sessions will take place within Club Pilates-Arbor Lakes at 11658 Fountains Drive, Maple Grove, MN 55369. There is a small waiting area directly inside the door. Please wait there until your provider is ready for you. There are 2 entrances to the building--please enter through the East entrance (nearest the parking lot, and the Costco gas station).
